New Trick for PDF Creation of Complex Webpages & Layouts on ios

Introduction

Creating PDF’s for Difficult to Manage Webpages Can Be Challenging

Most of the time when I need to create a PDF, it’s either in the spur of the moment, or it’s on the fly. I use Apple’s built-in (but still somewhat secret Print Screen PDF creation utility) which I describe in detail in this post.  But sometimes that simple method doesn’t work out well and the resulting PDF is either laid out awkwardly or it’s almost completely unreadable. When that happens I usually move up to a higher level app…a paid ios app by Readdle Inc. called PDF Converter which I also describe in some detail in the aforementioned post.

The Best Way to Save Webpages to Apple Notes

If you’re wondering what I’m doing with all of these PDF’s, it’s how I store data within Apple Notes. My other primary notes app is Evernote which offers a handy Web Clipping utility that allows you to select and save virtually any information from almost any webpage that you may encounter. In comparison, Apple Notes is pretty archaic!

Archaic that is until you discover Apple’s little PDF creation trick in ios…Using the Print Command. While it doesn’t provide all of the flexibility of Evernote’s web clipper…it provides quite a bit more than is initially apparent. By far, using Apple’s PDF trick is the best way to save content from webpages directly into Apple Notes. It just so happens it’s the fastest method too! It’s not only fast but it’s a widely used method too. Therefore it functions well and has survived well through several iterations of iOS.

But, sometimes a webpage has a complex layout or it’s formatting is too convoluted graphically for even Readdle’s PDF Convertor app to work well. Today I discovered a great work around using Readdle’s app when that occurs.

Example|  Trying to Create a PDF of a Difficult or Uniquely Formatted Webpage

Below are 3 examples which show exactly what I’m referring too. The first one (#1) is a PDF I created using Apple’s built-in print utility for ios. The PDF is a map of Kruger National Park in South Africa, created from this page which I’ve linked to here: Kruger National Park | South African Safari & Lodging Guide Created Using ios Utility

This is compared to a similar PDF that I created using Readdle’s app after I realized that the first PDF was useless. But the second one also was useless.

In both cases, the way the webpage is laid out prevents the creation of a useful PDF because most of the content goes off the right edge of the page and is unreadable.

While I was within Readdle’s app trying to figure out how to salvage a useable PDF from the site I noticed something I’d never seen before. A menu item listed simply as ‘Clipboard.’

The Clipboard menu item

Upon further exploration, I discovered that I could use this feature to copy the whole page onto the ‘invisible clipboard‘ and then have the app convert that data into a PDF. When I tried it I was pleasantly surprised to see that I was finally able to get the webpage for Kruger National Park cast onto a nicely formatted PDF!

See Example 3 below for the final useful PDF I created using PDF Convertor’s Clipboard Feature

As you can quickly see in the accompanying illustrations, the only PDF that’s legible of the 3 is the last one…#3.

How to Use PDF Converter & the Clipboard Option to Create a PDF

Go to the troublesome website and attempt to copy the entire webpage onto the  ios clipboard. If ios offers a Reader View, copying this will probably be easier.  An available Reader View is indicated by five lines in the address bar as shown in the screenshot below.

Regular View and Reader View of a Wikipedia webpage

Once the webpage is copied onto the clipboard just open PDF Converter and tap on the ‘Clipboard  Content ‘ menu item on the left.

If all goes well the app will create a much better looking PDF. One that includes all of it’s text in a readable format like that shown in Screenshot #3 above!

Modern Medicine’s Most Amazing Miracle…In 2018 Patient’s Can Achieve A Complete Phase 1 Recovery in Just 9 Days When Minimally Invasive THR Total Hip Replacement Techniques Are Used

Part 1 of my Hip Replacement Series

This is Part 1 of my Total Hip Replacement Series. Part 2 talks about the things I got to make life easier post operatively, what I used to help me heal quickly and the things that helped me most in coping with serious leg cramps in my surgical leg. Both parts are being written for a sibling who’s undergoing a THR in about a week. Part 3 may or may not happen so I won’t waste time describing it here unless it does!

Introduction

The miracles of modern medicine extend far beyond the relatively simple confines of total hip replacement technology…yet it’s that procedure, (formally known as THA or Total Hip Arthoplasty) which continues to astonish, amaze and even inspire thousands of Americans to place their complete trust and their future’s well-being into the hands of a total stranger.

As hip implants have been refined and the techniques used to implant them have been tweaked and modified, overall success and patient satisfaction have sky rocketed. We find ourselves in 2018 at a pinnacle of modern medicine. As it tends to happen every so often in the world of medicine, once again the full reality of its miracles can’t escape our observation.

My own personal evidence? Following a right-side total hip replacement 9 days ago, I wasn’t entirely prepared for an experience which surely has been nothing short of miraculous, if only from a purely logistical perspective.

Living My Own Personal Miracle

 ______________________

9 days ago I was admitted to a local hospital for a complete hip replacement.
Today…exactly 9 days later…I was cleared for any and all activities including driving!

Despite the fact that my right hip was the one that was just replaced! During the course of writing this, which began on the miraculous Day 9, I had more ups and downs than one might think possible! That explains in part why I’m finally publishing this on Day 13. Despite my early Day 9 love fest…and subsequent downs…and ups…I know that my progress is moving steadily up, and each day I have some new improvements which combined, have made this overall experience more positive than negative.

 A Matter of Miracles

A  Slightly Meandering Thought

I am truly my Father’s daughter. Shortly after reading this my 92 year old Dad, who somewhat miraculously still writes often at his own website PotPourri, shared with me an essay he wrote several years back, which I had completely forgotten about.

74CBC580-5EC9-474C-A93B-04B0EA4C887D

It’s somewhat surprising that I did forget since his essay,  A Matter of  Miracles is perhaps one of my all-time favorites, if for no other reason than the fact that my family and I are one of the central themes in his story. But it’s just a favorite story too. At the risk of sounding annoyingly obtuse…just go read it already…it will warm your heart and make you smile.

But what really makes me smile is how much Dad and I love to find the amazingness behind any simple idea or construct. The sheer joy we share while marveling at some of life’s wonders, like this YouTube video Dad shared with me today about Amazon’s robots…it’s like no other, and no one could appreciate this like we do! It makes me realize that I could not have been easily dissuaded from writing about the miracle I got to personally experience in such a profound and meaningful way ;-)

Above: My Before & After X-rays. A Hip Replacement Involves Removing A Large Amount of Bone That Comprised the Original Hip Joint and Replacing It with a Titanium & Plastic Prothesis

How Did I Arrive at the Point at Which I Needed a Complete Hip Replacement?

A Brief Orthopedic History

My history of hip problems was a lifelong one. The moment I was born my Mom’s obstetrician noted to my parents that I needed to be taken for an evaluation of my hips. That was done and it proved to be inconclusive. But it explained why my parents weren’t especially alarmed when my ability to walk was delayed significantly because my right leg ‘buckled under’ me every time I got going.

Better late than never, I finally did manage to wrangle control of my errant right leg enough to successfully walk. But that same leg continued to try and mess with me and my life for the next 60 years.

Some of the ways it did so were by causing my gait to be so off that my Mom took me to an orthopedic doctor who diagnosed me as having Perthes as a youngster. It was just a ‘mild case’ of Perthes he said.

Perthes, is a childhood onset hip disease that a friend and classmate also had. But my friend’s case was a much more serious one which required him to remain in a complete body cast for an entire year!  He was around the age of 9 years old at the time.

I truly hated the ugly nude-colored corrective shoes that I was forced to wear for years following my doctor’s opinion and dx. But, since my  friend and fellow Perthes sufferer we’re both undergoing our diagnoses at almost the exact same time…  quickly placed in their proper perspective when I found myself carrying around the little receiving end of the electronic box which our phone company had installed in my classmates dining room. His family’s dining room would be his home for the year that he lived in a body cast.

Although fate stepped in that time to make sure that my misery was properly mitigated, later on, as a teenager wanting to keep up with my peers in sports, I wasn’t quite as lucky. Mainly because I was constantly frustrated in not being able to control that wonky right leg very well.  But, in general, I discovered that with time that enough to do most of the things that I deemed important.

This historic image from 1969 documents the first total hip replacement performed in the U.S., a technique developed by Mayo Clinic Department of Orthopedic Surgery. Development of the total knee replacement technique followed in 1971. These successes ushered in a new era of hip and knee health. I was 12 at the time and my overall health was good…primarily because of Mayo’s!

Mayo's in 1969 is Generally Regarded as the 1st Modern Day Hip Replacement

Their doctors in Rochester had performed a different kind of miracle by bringing me back for the brink of death just a few short years earlier when I suffered from a random and bizarre case of total kidney failure. This followed a bad fall I’d taken on the playground which had resulted in badly scrapped knees that ultimately  became  infected with Strep B…leading to childhood Nephritis…a serious childhood kidney disease.

Things like snow skiing (downhill) and water skiing (I was part of a ski show) for example were sports I excelled in when performing left-sided maneuvers or even making left turns. But my right side was a completely different story. Probably the hardest part was my lack of understanding of the causative effect which my wonky leg played in these seemingly dramatic childhood and teenaged events. It was years later as an adult and a parent, that I began to put the pieces of this puzzle together correctly.

Although truthfully, I have to give most of the credit for this insight to an amazing downhill ski trainer who could see the blatant differences that I could only sense were happening.

Surprisingly, even though a constant theme in my life was this oft-repeated series of accident prone mishaps leading to casts, crutches, boots and ultimately surgery, which sort-of forced the issue…the abnormalities I lived with for 60 years were never really given a name. Hip dysplasia and Perthes, while bandied about, didn’t really fit as a proper diagnoses.  So I gave it my own diagnoses…which is how the wonky leg descriptor came into being.

You can see in fact that my description didn’t really consider my hip to be the sole bearer of all the problems I experienced. Responsibility was equally shared with a wonky foot and a wonky ankle too. Later in life a wonky knee joined forces. That’s why, when my foot surgeon told me that he thought that I needed a total hip replacement I was in a state of total disbelief. This was something that had never been mentioned before…even as a remote possibility. The fact that the leg of my surgical foot was pointing in a 90° angle away from its proper direction, that it stubbornly refused to be forced back into its proper alignment, and that it had begun something which I imagined closely resembled my early childhood ‘buckling’ problem notwithstanding…my immediate reaction was one of shock and disbelief.

What Makes Hip Replacements the Modern Day Miracle of Medicine in 2018

First it’s Because Present Day Hip Replacements Are Very Different from Their Predecessors

Almost every aspect of this surgery has been optimized from its 1960’s origins.

Advances in anesthetic techniques make the procedure much less painful while decreasing side effects and dependence upon opioids, which allows patients to walk more quickly after surgery (usually within 4 hours) and to leave the hospital earlier (often the same day or next day.)

Improvements in procedures which tend to cause less internal damage and use smaller incisions means that healing times are greatly reduced.

For example, many of the newest minimally invasive, muscle sparring procedures used today may require much greater surgical skill and technique, and sometimes even specialized instruments…but their resulting shortened window of rehabilitation makes them attractive to younger generations. As a result there’s been a significant increase in the overall number of procedures performed. The average age of patients undergoing joint placements including both hips and knees is decreasing too. Patients in their 20’s, 30’s and 40’s…once almost unheard of, are commonplace today.

So these new procedures, coupled with a lot of advances in the reliability of  the prothesis that are used today are what’s moved this type of procedure from the medical marvel category to that of miraculous.

2nd it’s because people who would once have been crippled by arthritis for the rest the of their lives can now go on to lead normal, active and highly productive lives.

People who are younger are increasingly undergoing both hip and knee replacements, and some recent studies are even showing that these types of procedures seem to increase overall life expectancy by a small amount.

When I was researching the type of procedures that are used today I encountered roughly 30…but those fall into larger general categories which Mayo’s wrote a nice overview of last year that I’ve shown below.

Mayo's overview of hip replacement procedures

The type of hip joint implant which utilizes metal stems and polyethylene cups or the newer highly cross linked polyethylene (HCLP,) which are afixed to the bone without cement, appear to cause the least number of long term problems by withstanding unexpected wear better and exhibiting less part failure, and less long-term biological implications. Also Ceramic joint surfaces can reduce friction and avoid the chance of metal corrosion in the joint. The combined results of these improvements mean that a new hip joint should function well for many years…quite possibly much longer than the original estimates of 10-15 years.

This is especially important for people in their 20’s, 30’s and 40’s…who have the most to lose from typical implant problems like wear particles being distributed internally and tissue irritation like pseudotumors. This 2014 article does the best job of describing those types of problems by implant type.

 B. Sonny Bal, M.D., writing for eMedicine wrote a great overview of the entire minimally invasive hip replacement process pictured below and found here.

Not Your Grandma’s Hip Replacement

The ‘miracle’ aspect of modern day hip (and knee) replacements didn’t really hit home with me until I read this enlightening short blog post by a guy who’d just undergone cardiac surgery. The observations he made while still admitted about 2 other guys who we’re there for total hip replacements helped me to recognize how truly miraculous these new procedures are.

Fast Forward Back to My Present Day Decision to Undergo A Total Hip Replacement

Pre-surgery Preparations

Once my mind was made up there was no looking back.

I’d been preparing for surgery for weeks, and was frankly a little bit surprised at how much preparation was required. Through a series of online tutorials which I received daily emails from my surgeon instructing me to visit, I was instructed that I needed to get our house ready, myself ready and my life suitably placed on hold.

All told, the preparation took about a month. I approached the class, educational resources and the daily online checkin’s my surgeon required very seriously. I followed their instructions to a T (almost.) I diligently worked out specific muscle groups surrounding my hip joint. I removed any and all small rugs, cables and impediments which might cause me to trip or fall post-op. I prepared a place to sleep downstairs (which involved getting some new, inexpensive furniture,) and I practiced with my crutches so I’d pass the hospital’s test and wouldn’t be required to bring home a walker like most new hip recipients.

The Day of Surgery

In spite of all the preparations, I’m not gonna lie…the day of the actual surgery was a bit tense. But then the procedure went well and it only took about an hour an a half. My surgeon used an anterior direct approach, which is considered to be one of the more minimally invasive, or muscle sparing procedures…but perhaps not the most. I discovered there are 2 main approaches…anterior or posterior…but roughly 30 modifications of these 2 main approaches that each involve slightly different rates of recovery. It wasn’t easy learning what my expected rate of recovery might be, and in fact I never really did!

No one ever talked to me in detail about that part. So I was left to assume…and my assumption was maybe a month. The specific type of procedure performed, which might have the most significant impact upon recovery rate, was never guaranteed. Because you never know what might crop up at the last minute. So I guess I was somewhat lucky that the original plans were able to be executed as intended in the final ‘actual‘ plan.

Even so…remember no one told me what to expect in any kind of detail. Probably the person who came closest to this was my physical therapist who I met with only once  pre-surgery. His description was vague and not specific because I didn’t know until that meeting how much I really didn’t know about my own procedure!

Also, vagueness was probably due to the fact that there’s a huge amount of variability in recovery rates, based upon a patient’s age and overall level of health as well as their overall level of physical fitness too. Perhaps because my own physical fitness level had been less than stellar…primarily because my hip prevented me from doing so many things…my expectations were low.

That’s why…on Day 9, towards the end of my first physical therapy session I’m was literally stunned to learn that I’ve been given the ‘all clear’ for just about everything!Me, Happy I'm Given the All Clear

Here’s What I Can Do at 9 Days Out

  • Walk unassisted without crutches or a walker. But everyone wants me to continue to use my crutches for just a little while longer…just to make sure.
  • Go up and down stairs the normal way…I should be careful however…especially going down. I should probably also use the ‘easy way‘ with a crutch for a while…just to make sure. (The ‘easy’ way involves taking just one step at a time, landing on each step with both feet instead of just moving quickly from step to step with each foot.)
  • I can drive!!! Since it was my right hip that was replaced I find this nothing short of a miracle! However, once again I’ve been advised that I should take it easy at first. Maybe even take a few short practice runs first…which given my driving record makes a lot of sense!
  • In short I can do much more now than I could 9 days ago. 9 days ago arthritis prevented me from doing many things. Some as simple as sitting in a car, others more taxiing like running. I didn’t think to ask if running is advised yet but given my physical therapists general instructions of do what I want letting pain be my guide, I imagine I could if I wanted to.

Here’s What I Can’t Do at the 9 Day Milestone

  • Lunge – I was cautioned about this type movement upon being discharged from the hospital.
  • Jump & Hop – The only reason I know this is because my ever humorous husband (and my rock throughout this process) did ask at my 1st PT session today.
  • Have a Dental Procedure…I need to wait 3  months and when I do I need to take an antibiotic first.
  • Sleep – But not for obvious reasons. In fact, sleep is generally encouraged….I simply can’t do it.

Lifting My Leg is Still Difficult

My Inability to Sleep is the Only Real Problem I’ve Experienced

Its kind of a big problem though. What’s preventing me from sleeping is cramping and charley horse types of pains in my surgical leg. In fact, overtime, what began as just nighttime pain has become pain I experience almost all day long. The pain is a moving target. It can occur in obvious places like my hip, groin and buttocks, but in less obvious ones too, like my knee, calve and ankle. One reason this presents a bigger problem is because of Deep Vein Thrombosis, which is a significant risk following any hip replacement. DVT symptoms resemble the symptoms of the leg cramps I’m experiencing too.  So, while there was this undertone of worry initially by my surgeon, I was pretty sure that I didn’t have DVT, and that fact was confirmed by an ultrasound.

What this does mean however for someone else who like me may also be prone to restless leg syndrome or nocturnal cramping, is that they too may experience these symptoms. They may become more profound at night, and so far at least they’ve proven very difficult to treat.

My PT guy believes there are due in large part to the amount of trauma that the surrounding areas underwent…cartilage and tissue damage I’m guessing. He also believes they may emanate in part from a significantly different gait that I’ve assumed post operatively.

CDFC796A-03FB-420F-A9D2-B5C9995E6CA5

Why My Postoperative Gait Might Be Different

One thing that no one mentioned to me and that I also failed to discover during my own research is that limb length discrepancies are fairly common following Total Hip Replacements. Most often the surgical leg ends up longer than the other…at least initially.

Mine was and appeared to be almost 2″ longer than with my former hip. But this amount has already decreased and should do so even more in the future. 2 Weeks post surgery I’d estimate the disparity to be about 1″ now. There are multiple reasons for this. First is that many patients experience a sort of general ‘settling’ of the hip joint. Next, the surrounding area may remain swollen for a long time prior to finding a new permanent position. This swelling could obviously result in a temporary length discrepancy. Last, the leg can also just feel longer because the amount of erosion from  arthritic processes may have decreased overall leg length overtime…and the new prothesis has actually brought it back to the correct original length.

Below: My prothesis is made by DePuy

Below: My prothesis is made by DePuy

While the explanations for longer surgical leg length make sense…they don’t do much for dealing with the pain resulting from it. I’ve had a solid week to experiment and the cramping and pain is some of the worst I’ve ever experienced. My former remedies have failed to yield positive results. So, I thought I’d include a small section on what I discovered did help me tremendously in coping with this pain. Because pain management post operatively is stymied by patient’s ability to use so many of the solutions which would otherwise be obvious…but which currently are not allowed.

Things like NSAID’s and even NSAID topical treatments aren’t allowed due to potential post op bleeding. Another soothing activity which is prohibited currently is soaking in warm water or whirlpools…because the incision (which I haven’t seen yet but  is protected by a massive sized 12″ bandaid) can’t be submerged.

My Recommendations for Leg Cramps While Sleeping (and While Awake too!)

The Soheryii Air Compression Leg & Foot Massager from Amazon for $129.99

I’ve run through the full gamut of past proven and folk remedies for the nocturnal leg cramping problem…everything from Hyland’s Homeopathic Tablets and Ointment to Topical Magnesium, calcium citrate, mustard, dill pickles and electrolyte beverages like  Gatorade.  Many help a little but none work well enough to stop this most stubborn, persistent, and almost continuous problem. It began just at night but worsened to all day, everyday.

Only one thing has given me consistent relief…and I’m as surprised as anyone to find that it works for me!

While in the hospital they employed these inflatable leg compression devices on my lower legs which combined with compression socks were intended to prevent blot clots and deep vein thrombosis. They worked well and I got compression socks for home use too.  It in searching for the hospital’s compression system I found it was $5000…not worth the cost for home use. There are others on Amazon and the main price categories tend to be around $300 or around $1000 for full leg compression..,which I wanted. But I couldn’t find any of the around $300 units which would arrive at my home soon enough to be of use for me. The earliest would have been about a week out (it’s St. Patty’s Day right now.)

So I ordered that set knowing I could return it and called Amazon Customer assistance. They assisted me in finding this less expensive set for $129.99. I had seen it but thought the reviews weren’t promising and it wasn’t one complete leg unit but multiple units which looked confusing, had a lot of cords and was potentially cheap and nonfunctional.  But I did order it because it was available for delivery the next day.

Putting it together and using it initially were beyond confusing! It truly took both my husband and myself to figure it out and get this unit working. But our perseverance paid off big time…because this one device has been a Godsend…and it’s the only reason I’m now sleeping through the night!

Below: The Amazing Leg Compression Device   Which is Helping with all My Leg CrampingThe Amazing Compression Device which is helping me sleep!

A Few Reason’s Why The Soheryii Air Compression Leg & Foot Massager Has Been So Amazing

As I mentioned…I thought I wanted one long leg sleeve…but it turns out my pain is constantly moving…and by having the different sized pieces I can place them exactly where my pain is. Second…I read that a lot of the more expensive units had such strong pressure they were more uncomfortable than relaxing…I needed relaxing. This one has a default setting of 5 but it goes up or down from there. I generally go down to 2 which is perfect for me. Third…the long leg parts zip…the shorter leg cuffs use strong velcro.  I can unzip the zippers some but it’s not as effective as readjusting the velcro positioning. Therefore the velcro allows me to customize the pressure even more than the electronic control does.

I use these massagers every time I sit down, because leg cramps begin within a few minutes of sitting down. What happens for me is I’ll be sitting with my legs extended and elevated and some ice packs on and feel comfortable….say maybe for half an hour…and then I move. That’s when the massive cramping sets in. I can’t move, walk or do anything for the next few hours without massive leg cramping. So if I’m walking around doing stuff…which I do a lot of because I have a lot of work to get done…the pain ebbs and flows with my consumption of Gatorade, dill pickles…etc. If I’m sitting I just put the massagers on and maybe some more ice and I’m good to go!

A Few More Leg Cramp Remedies I’ve Found That Are Helping To Resolve Them Permanently I Hope

Because The Cramping Isn’t Getting Better On It’s Own

In the past I’ve had some success with homeopathic remedies…so I decided to try everything I could find. Hylands makes both a tablet and a topical cream that’s helped me some in the past…I got both and feel the cream is more immediate this time. Another homeopathic remedy for leg cramping in tablet form is made by Boron, and is called Cuprum metallicum and it comes in both 30c and 6c strengths.

A great homeopathic remedy for any kind of injury or trauma is Arnica. Arnica promotes healing and it comes in tablet, cream and gel form…I’m using the cream and tablets. Trumeel is yet another homeopathic remedy I’ve found which works well for pain management. Unfortunately you can’t get it in the states anymore, which I discovered the last time I wanted to refill it. Amazon did have a foreign source for it but it’s expensive and slow to arrive. But I did re-order it sometime back a year or 2 ago…mine came from Russia…and I’m glad I did…because now I have it to use. Trumeel is somewhat unique because it also has anti-inflammatory properties too.

Last I tried an ultrasound machine…without much effect, and I just obtained aspercream rub after reading that it helps too…but I haven’t tried it yet.

Phases or Levels of Healing Best Define the Long Term Process for Rehabilitation Following Total Hip or Knee Replacements

The way the human body heals can be thought of as a process which begins at a rapid pace on a slightly superficial or outward realm. It’s rate slows down as healing moves inward to much deeper, more substantial levels…ending at the point upon which the structural integrity for the whole mechanism in question…be it a new hip or knee resides.

Therefore, my miraculous 9 day recovery while primarily a superficial one, does hold up under closer scrutiny as well. What’s missing can probably best be described as   ‘strength.’  Over the course of the next few weeks and months, my aches and pains should lesson while my bone and tissue continues to grow and gets stronger. Everything should begin to meld and form a new normal for my brand new hip joint. This process should near completion within a year to 2 years. So, what the minimally invasive moniker really refers to is a significantly less intrusive early phase which allows patients maximum uptime time and minimum loss of productivity.

Some Additional Interesting Facts About The Modern Day Medical Miracle of Hip Replacement Surgery

1969 was the year in which Mayo’s physicians (from Rochester, Mn.) undertook their first total joint replacement procedure…they chose a hip replacement as their test case. It took 2 more years to perfect the technique enough to perform a complete knee replacement in 1971.

Although the first THA was performed by Mayo’s physicians, Sir John Charnley, a British orthopedic surgeon, is widely acknowledged as having developed the fundamental principles of the artificial hip and is credited as the father of THA.

Success breeds popularity…total hip replacements in 2018 have never been more popular. Roughly 300,000 of them were performed in the United States alone in 2017.

If you’re interested, Dr. Dunn, a retired surgeon recently published a fascinating video overview which covers the entire history of THA’s, with a focus on finding the best prosthetics, in January 2018. But it’s not for the feint of heart. Learning about some of the problems with these implants can be frightening if you’ll soon be using one personally.

One More Medical Miracle I Lived to Experience

I don’t remember the actual sequence of events, and how the unfolded, but my dad wrote a story on his website about how Mayo Clinic save my life when I was seven years old.  The part that’s confusing to me is how I came to write about it on my sister website vsatrends.  But it is a great story so here’s a link to how I almost died and how Mayo clinic save my life when I was seven years old…told from the perspective of my father.

Part 2 of My Hip Replacement Series

Part 2 is much less about the story behind my hip replacement and much much re about the uts and boots of what I used to survive the first few weeks. Because I had a lot of ‘anciallary’ pain…brought about by the surgery, but not directly due to it, my surgeons’ office didn’t really help much to address it. Nor did the pain meds I stopped taking relatively early for that exact reason.

I’m a big believer in alternative therapies. If traditional medicine can’t address a problem then I let alternative resources take a stab at it. This approach has worked out well for me in the past and did so here too. But it’s a lot of ‘trial and error’ and which is something my OCD focused mind excels at. So Part 2 is a long and exhaustive treatise of what did and didn’t help me or work for me. Ranging from homeopathic remedies to nutritional supplements and even diet!

Keep in mind too, that what worked for me my not work for you. However, it gives you many alternatives where before, there appearred to be none.

If you or a family member or close friend isn’t undergoing a total hip move r knee replacement I doubt Part 2 would interest you, unless you have an interest in learning more about natural, herbal or homeopathic solutions for injury and pain.

Today is the Day YouTube Changes Forever…could the internet be far behind?

Update: Sept. 2018
Youtube finally responded to all the small channels they kicked out…sometime in the spring or summer. I need to research this bit more and I’ll update here. Also, its unclear if smaller channels that aren’t Youtube ‘Partners’ are truly banned from using Patreon and having sponsorships…another thing I need to research!

Because YouTube & the World Wide Web

Heralded in a new period in history

Where Community & Collaboration Count

________________

Hello to all My Readers, Followers & Subscribers…

(and yes, each of those really are different & really matter,)

You may (or may not) have already heard about how the YouTube management team has decided to drastically change their widely popular, ever entertaining video sharing service. As a result, the amazing community of people who arose from those roots…roots mind you, that YouTube planted…the very people who love creating & sharing their videos the most…known world-wide simply by the moniker YouTubers…are either already gone or they will be gone very, very soon.

It will be a mass exodus the likes of which the Internet has never seen before.

You, in an outraged, yet disbelieving tone of voice…ask…

Wait…What? Why?

The short answer my friend, is that immature corporate leadership bolstered by unseasoned yet inexplicably arrogant ranks recently realized that yes, in fact profits really do matter after all.

Statistics and Image Credits:  Think Media VideoThink Media's Video https://youtu.be/qQ5GI4p30_8

The longer answer is exactly the reason why I’m writing this post today.

But…if you want to skip all that here’s another short answer…

Because YouTube has finally decided that Profit is their most important and in fact their only measure of success going forward. Of course, that’s not what YouTube Corporate will tell you. Naturally Google Corporate won’t either. But after you read the evidence presented here, you can decide for yourself.

Without a doubt, YouTube has broken all records in terms of popularity, growth and even one that may shock you to learn:

YouTube is one of the Biggest Employer’s in the World!

Perhaps one of  the most clueless too!

Basically because, they don’t even know that they are one of  the  largest  employers!!

Who would have guessed that? That’s because Google (aka YouTube…they are essentially one in the same…because Google owns YouTube,) recently decided they no longer want to be in the business of having thousands, if not millions of livelihoods dependent upon them.

Was this because they were worried about shouldering the great burden of responsibility that this entails?

That doesn’t appear to be the reason. Google and YouTube have publicly stated their reasons…but those don’t make sense to anyone. They talk in circles about factors like ‘protecting their audiences from bad actors.’ Reading between the ‘not so murky lines’ however, most everyone has deciphered that to really mean that Google is protecting themselves against the possibility of losing advertisers…because advertisers are the only community that YouTube is concerned about.

What…YouTube Only Cares About Advertisers? Where’s your proof?

The proof of this is easily ascertained by anyone willing to take an hour or 2 to look at what’s transpired in the last year.

Here’s one of the most significant highlights:

In late 2017 and early 2018 Google was forced to reprimand one of their most profitable content creators, a completely self-absorbed twenty-something who goes by the name of Logan Paul. Not just once but twice!

CBS News This Morning Coverage of YouTube’s Questionable Actions

Huh? Why’s that Even Important?

Because these actions go to the heart of what’s happening at YouTube. Advertisers have grown utterly frustrated that YouTube allows their ads to be placed on videos like the controversial ones Logan has put out recently.

What Controversial Videos are You Referring to?

2 videos immediately come to mind. Logan published a video (shown above) in which he and his friends had the poor judgement to shoot and share footage of a young suicide victim’s fresh corpse they found hanging  in Japans ‘suicide forest’ while looking for scary or haunted things to shoot. When they ran across the corpse of this young man hanging from a tree limb its obvious they were seriously shaken. But that doesn’t account for their decision to immediately begin shooting video footage of the incident which they then proceeded to publish on their Vlog shortly thereafter. Mostly they were serious and horrified about what they found, but there were a few clips of them laughing and acting goofy. Since I don’t watch the channel I have no idea how sincere they really were about their shock and horror.

Their young audience, consisting mostly of children and young teens, was also horrified, scared and in a lot of instances profoundly disturbed by the incident. It was only after their equally horrified parents began voicing their outrage that advertisers and YouTube even took notice!

Quite a lot of time elapsed (at least a week!)  before YouTube finally addressed the situation. Their response was to temporarily take away a few of Logan’s revenue sources…leaving him with a few dents in his reported monthly profits of 1 million dollars. But the most important fact to be gleaned from this incident is that YouTube management DID NOT TAKE AWAY HIS CHANNEL OR EVEN HIS MONETIZATION OF IT!!!

_________________________

IN FACT…The Most Shocking Thing of All Was That YouTube Didn’t Even Ask Logan to Take the Video Down!  He Did That All on His Own Accord.

Shame on you YouTube! I literally just learned right before publishing this that it was Logan himself, and not even you who took down that awful video! omg! Any dwindling respect I had for you is circling the drain as we speak!

Monetization of a YouTube channel basically means that Google allows advertisers to display their ads on videos of that channel. Which in turn means that Google earns advertising revenue from those ads. When Google allows channels to monetize too, it means that they let the video creator also profit from the ads. The percentage that Channel Owners earn is quite small. But for channels like Logan’s, with his subscriber count of 15 million, all those tiny payouts add up to some very big dollar amounts!

OK…I follow you so far…but you said there were 2 incidents! What’s the 2nd one?

Right you are…and sorry…I got a little off track. So, the second incident needs a little bit of context first.

When Logan finally realized the stupidity of his actions he took what he felt were the necessary remedial actions to set things straight. He learned everything that he could about suicide and went on to produce an apology video which (glibly) teaches us all about the negative consequences of suicide. I say ‘glib‘ because it dawned on me that Logan might have been the only person in the world who really needed the kind of instruction he provided in his apology video. Most everyone that I know, from kids on up to the very elderly already understand what the negative aspects and consequences of suicide are.

Image 1240

I should mention that my telling of this story is done purely from my own perspective…although I’ve researched and read volumes on all these topics in order to present as objective a picture as possible…but there may be some minor details that I’ve missed because there are simply so many details involved.

So, my understanding is that following the world’s lukewarm reception to Logan’s apology video, he must have believed himself to be ‘in the clear’ or exonerated for all practical purposes (go figure!)  He resumed production of the daily Vlog he’d personally put on hiatus following his 1st reprimand from the management team at YouTube. I’m not sure how many days had passed, but it wasn’t too many, when Logan decided to stir things up a little more. He chose to do this by publishing a video of himself poking, prodding and tasing 2 rat corpses…in a misguided attempt at humor. Don’t forget that many YouTuber’s who reach such enviable heights of fame at such young ages oftentimes have abandoned their education in pursuit of their dreams for stardom (long before they should have!)

When YT management caught wind of this 2nd incident exhibiting extremely poor judgement they added a few more temporary monetary reprimands.

You Don’t Mean to Tell Me That…Once Again YouTube management failed their massive world audience (and their advertisers too come to think of it) with their 2nd set of relatively inconsequential Logan Paul reprimands?

I Absolutely Do! YouTube’s management ranks appear to be incapable of making that blatantly obvious decision. Their reprimands, which almost appear to many to be…well silly… seemingly stem from their avoidance of potential temporary corporate hardship which might result in a negative impact upon their robust bottom line.

OK, I do get it…seriously though…it doesn’t take a rocket scientist to figure out that the most immediate, the best and probably the only real solution to this problem is to shut down Logan’s channel…it should go without saying, permanently.

I know…right? You should be writing  this post!

Wow! That’s truly awful! Unless I’m missing something though, I fail to see how Google’s role as one of the world’s largest employers fits into this picture. Also, how can subscribing to YouTube channels make any kind of difference at all??

Well now that we’re on the same page…I’ll try to close all those gaps

The long and the short of it is that YT managements’ obvious shortsightedness has one additional horrific consequence.

But…that consequence is really hard to explain to any rational thinking person…so please bear with me for a moment longer OK?

OK

So, in addition to their failure to adequately reprimand the one guy who’s really responsible for this recent chain of events, YouTube took it upon themselves to issue one additional, but much farther-reaching reprimand. I’ve gotta be completely honest with you though…I think you’re going to have difficulties understanding this.

OK, go on.

Google, I mean YouTube decided that the creator community and the small channel roots from whence YouTube arose was in fact, somehow responsible (albeit indirectly responsible) for Logan’s transgressions.

So today, Feb. 20th, because I haven’t managed to grow my Subscribers from where they were on January 16th enough (my subscriber stats were at 90 and now they are at 115 but they needed to be at 1000)  (January 16th was the day of YT management’s announcement of the mass firings)…so, because my stats haven’t reached the newly mandated minimum of 1000 and  because I didn’t ‘increase’ my hours of viewed-time too, from 12,000 minutes to 24,000 minutes…or 4000 hours…(it’s worth noting here that our reports, provided by YouTube, only state our channel’s view time in minutes not hours…so quite a few people…including myself originally, checked our stats and believed that our channels would avoid this newest reprimand thinly disguised as an incentive)…well, I’ll stop beating around the bush already…we were all just fired..EFFECTIVE IMMEDIATELY!)

So, what you’re saying is that all the small channels are essentially being forced to pay for Logan’s transgressions with their own blood, so to speak. Have I got that right?

YES! You’ve completely grasped the situation (much more readily than  I did…I  might add!) So here’s the thing. YouTube says that the small channels it’s booting out don’t earn enough to make it worth their while…and while logically we get that…that doesn’t explain why YouTube is also going out of their way to make it next to impossible for us to earn income in any form whatsoever within the YouTube realm.

Why do you say that?  How is YouTube going to prevent you from earning a living on YouTube if your income source isn’t them?

Because it seems that YouTube has gone out of their way to directly block any viable source of income. Things like Patreon for example…if you’re unfamiliar with Patreon, it’s a service that lets people setup accounts and then contribute to their favorite content creators. It’s a pretty brilliant concept really. But apparently YouTube has decided that the small channels they are booting out won’t be allowed to use Patreon  either. Another source of income would be if a company likes your channel and wants to sponsor you, they could under normal circumstances do that…but not if you’re a small channel that’s being booted out right now. No small channel that’s being thrown out will be allowed to have sponsors either.

That’s why earlier I said that while YouTube glibly likes to refer to their new mandate as an incentive…when clearly it isn’t…it’s a reprimand plain and simple…but it’s a reprimand for something we had absolutely no part of or played any role in!!!

This is the reason that small channels are leaving in droves. We’ve been repeatedly lied to…or, at a minimum, lured into believing that if we would just work a little harder, and  knock ourselves out a little more to reach this weeks new random level of mandated activity…a level that YouTube has just ‘literally come up with out of the blue ‘ and decided to thrust upon us…then we’ll really be YouTube’s partner. None of the channels, large, small or in between, have any kind of influence or even consideration for their own set of circumstances…and they never will.

It Always Has Been and Always will be…

YouTube’s Way or the Highway

Many, many, many more channels than just those that are being booted out in this newest round are beginning to realize this fact. It’s a pretty impossible structure to build a business upon, and a pretty slippery slope to risk your entire business model upon.

Geez…I don’t even know where to begin and or what to say…

I know…right? What do you do with that? But now do you see why the YouTube the world has grown to love will end. It can’t continue if it’s most valuable resource, it’s creators, aren’t around to well, create? There’s one more aspect of this that you should know about too. That’s that YouTube is fully aware of this and frankly could care less.

YouTube no longer values individual creators. They value big business…the bigger the better. That’s why they went head to head against network television…to force their advertisers to leave them, thereby forcing them to ultimately have to join forces with YouTube. So now they have…but YouTube is their business partner…you know what that means now…right?

So the YouTube we all have grown to love is leaving us…but YouTube the institution will still be around…it will just be very different. It won’t be the unique, user-generated content of the past, but instead will be professionally produced programming similar to the kind of network television programming that older generations (like myself) grew up with.  YouTube TV is a perfect example of that.

Image 1242

Subscribers & More

I waited until the last possible moment to publish this thinking that surely someone else would. Someone bigger and better, someone with a much larger audience than ‘moi’…but it didn’t happen. No one did! Go figure? Maybe there’s more than meets the eye to my son’s argument than I’d originally realized. Let me explain. When I first mentioned my plans to my tech-oriented son, he somewhat endearingly blurted out…but…”what if you get Google angry…they could take away all of your Google stuff!” I thought about that a moment and then realized that while I do have a lot of Google ‘stuff”…there’s only one thing that I really care about…that’s my YouTube channel. They’ve already taken that away! So I have nothing to lose and everything to gain. Maybe others who are bigger have more to lose?

And yes, I do understand that they haven’t really ‘taken’ anything away from me…anything other than income that is. The income that my YouTube channel generated, which was substantial and an important part of my overall business model. That model relies upon multiple smaller income streams which, when merged together provided a reasonable living wage. But every part in that equation was equally important, and nowhere in my wildest imagination could I have predicted that after working for years to get my Channel earning respectable figures, YouTube would decide to take that away from me in one fell swoop!

Other than that all they’ve done is jack me around a lot in recent years. They have repeatedly changed the requirements of being their partner (which I only recently realized certainly means that no partnership in the true sense of the word ever existed :-/ …so it’s kind of ironic that they chose to use that exact word…partnership!

Then again…they have taken away my channel in my mind. They’ve completely and utterly taken away any trust I may have once held and any shred of dignity too. Because I’ve begun adding these little bits onto all of my videos asking the people who watch them to ‘please subscribe‘ which is something I’d always felt was a real loser thing to ask!

Oddly, prior to January 16th when we were informed of our upcoming firing with a 30 day notice, YouTube always communicated to us small channel owners that having a lot of subscribers wasn’t important…in fact it was completely irrelevant. So basing 1/2 of our partnership status on that particular statistic is sort of a ‘slap in the face’ too! Was that intentional or simply more misguidance?

Image 1239

What Does it Mean to Subscribe to a Channel?

Clearly, from my own perspective, that subscriber button has always been really obvious and with its big red letters it’s pretty hard to miss. So asking people to subscribe is just begging imo! Also, almost everyone in the universe understands what subscribing is, right? So it’s demeaning to have to explain it or to beg for it…see where I’m coming from?

however

Herein Perhaps, May Lie My First Incorrect Assumptiondo people really understand what exactly it means to subscribe to a YouTube Channel?

I’ll leave that question to be answered by you, gentle readers. What do you think it means to subscribe?

IMG_2938

Given that I had to actually look it up myself…I’m really interested in your answers!  I can tell you what I thought….which turned out the be 💯 % incorrect. I thought it meant that I’d receive an email every time some channel I subscribed to published a new video.

I also thought that it meant in some small way I was endorsing that channel and the content they produce. Because I thought that when I subscribed my subscription status was somehow made public for all the world to see.  I was wrong in that assumption too!

In fact I was completely wrong with just about every assumption that I held on subscribing to YouTube channels!!!!

OK…I can’t stand to keep it from you another minute. I know I said I’d wait to find out what you thought subscribing meant…but I can’t. I’ve never been known for my patience ;-|

Here’s what it means to subscribe to a YouTube channel

When you do, if you go to YouTube to watch a video…and then if you go to your ‘home page’ afterwards, there will be videos lined up on that page for you that make it easier to find and watch the newest content from the channels that you subscribe to. Because I rarely go to that ‘homepage’ for lack of a better word (I don’t think that YouTube has an actual name for it) I never really understood before that that’s all that subscribing means!

Subscribing is such an innocuous activity in terms of impact upon viewers, that I can’t see any reason why someone wouldn’t want to support the channels they like. It’s a simple act that can benefit small channels a lot!

_______________

So, if there’s any opportunity to salvage YouTube at this date…and I don’t know that there is…I would like to encourage…no, make that urge, all of my readers, followers and subscribers…and anyone else reading this too for that matter…to go and subscribe like crazy! Subscribe to all of the channels that you Love, Like and heck even Dislike if you have any interest in preserving them. If you can’t think of any channels that you care enough about to subscribe to and you still want to do your bit…you could always subscribe to moi…I’ll even promise to quit referring to myself that way if you do (sadly dashing all notions of myself as bilingual in the process :-)

Here’s my Channel, creatively named  vsatips, because it was originated as a way to sometimes better demonstrate concepts that I’m writing about here.

My channel

Postscript

I have just one more tiny little thing to add. My son…the one that I mentioned earlier thought…that this was a crazy and impossible notion. Because I was thinking that I’d like to create a new community for all these channels that are being driven away from YouTube. A community that would resemble the old and familiar YouTube from the past. I even came up with a pretty brilliant (I think) name for it. We’d call it…

NewTube

I’d be more than willing to put in the time and invest as well. I’ve read a lot about how YouTube got started and in fact even have written about it. How hard could it be if three young college drop outs could do it? We’d need servers and coders…worst case scenario we could use Amazon AWS and Mechanical Turk to get started. And last we’d need lots of small channels! I know where we could find some of those :-)

A couple of interesting points to make include:

  1. When YouTube was first getting started their biggest hurdle was finding content and content creators. They ended up running ads on Craig’s list asking girls to upload a video of themselves and in return they’d pay them $10. They had no takers for their offer!  YouTube has just made our lives easy in that regard! YouTube, in 2018, has roughly 1.3 billion users. Every user is also a channel. That’s 1.3 billion potential content creators! I’m telling you…this thing could be huge just like its elderly relative is!
  2. YouTube’s early growth was so phenomenally fast (after a slow first few months)  that it was only about a year and a half  after those 3 guys… Jawed Karim, Steve Chen and Chad Hurley began it that Google approached them with their offer to buy them out…for 1.65 billion in Google stock…it was the largest purchase that Google had ever made!
  3.  YouTube first began in 2005  (it certainly seems longer ago than that to me!) Google purchased them in October 2006. They paid $100 a month for their first Server rental.
  4.  Everything about their story seems to me to be readily reproducible.
  5. I’d even venture to guess it would be significantly easier the 2nd time around because we’d have the perfect example to follow.
  6. I’d venture another guess to predict that we wouldn’t necessarily need to copy YouTube…we could innovate far beyond their ability to do so…because they’ve grown too large to be innovative anymore.  Also, because we could learn from their mistakes and stack the deck much more in our favor.
  7. 2 months after Google’s acquisition Time magazine put ‘You’ on the Cover essentially honoring YouTube’s role in bringing a new community focused kind of collaboration to the World Wide Web.

What’s the Best Way to Keep Your iPhone Cellular Bill Low When Traveling Internationally?

Introduction

Take it from someone who just received a $500 AT&T bill for an afternoon of sightseeing in Montreal. You need to know about one somewhat deceptively named  iPhone setting that could cost you a small fortune.

What’s WiFi Assist?

To understand exactly what WiFi Assist is I need to explain another somewhat similar iPhone setting that is easily confused with WiFi Assist. It’s called WiFi Calling. AT&T was the first cellular service to offer WiFi calling, only on iPhones initially, several years ago.

What’s WiFi Calling?

If you have poor cellular service in your home, oftentimes it’s due to things like construction features or geographic features like tall trees which make it difficult for cellular signals to travel well throughout your home. Our home happens to be fairly resistant to most cellular services…regardless of the provider. In the early days of iPhones AT&T provided us with a Microcell which essentially shifted our cellular calls over to our WiFi.

Read Apple’s Support Page About WiFi Calling

Image 1213

As technology improved and WiFi’s obtained higher bandwidth, Microcells became obsolete and cell phones themselves performed this same feat with WiFi Calling. Now, in 2018, most cellular providers offer some form of Wi Fi calling and most Android phones have it available as well. How to Geek has written a great explanation of WiFi calling.

WiFi calling is great because it’s essentially free. The only minor annoyance with it is that the seamless handing over of your call from WiFi to cellular if you leave your domicile while on a call, isn’t really all that seamless. What’s even worse is if your iPhone has this enabled and you’re not on a call…but you make one within a few minutes of leaving your home, you might find yourself unable to make a call at all that’s of any decent call quality.

Many times if I call someone from my car right after leaving home, I’m unable to hear them and vice versa. This can happen repeatedly until I turn off WiFi calling. But you’d be hard pressed to find any documentation of this problem anywhere! Just take if from me…if you use WiFi calling and frequently encounter poor call quality shortly after leaving your home…it’s due to WiFi calling being enabled.

Read Apple’s Explanation of WiFi Assist

Most people think WiFi Assist is the opposite of what it really is

Back to a WiFi Assist

The reason I believe WiFi Assist is somewhat deceptive is because the name of it sounds so similar to WiFi calling. Many people, including myself, don’t really realize that these are actually 2 separate features which function very differently.

Think of WiFi Assist as being the exact opposite of WiFi calling. It’s intended for situations in which you have little to no WiFi connectivity available to you. So instead of using WiFi your phone switches over to using your cellular service for things like Safari or other apps which require Internet connectivity.

You’re probably beginning to see the big problem with WiFi Assist already. If it’s enabled when you’re traveling internationally, your phone could rack up huge ‘out of carrier’ charges if you’re doing things like using Maps in satellite view or engaging in other data intensive pursuits.

The Default Setting for iPhones is WiFi Assist is On

WiFi Assist Uses Cellular Network to Boost WiFi Connectivity

WiFi Calling Uses WiFi to Boost Cellular Network Commectivity

My $500 Bill for an Afternoon of Sight Seeing

Our $500 dollar bill actually resulted from 2 situations we were unaware of. The first was having WiFi Assist turned on and the second was that the offending device  was my iPad mini, which also has a SIM card in it.  We knew and had in fact confirmed it multiple times during our travels with AT&T that our cell service was unlimited. What we didn’t know was that tablets were excluded from our unlimited plan. It was this combination of factors which was responsible for our $500 bill.

Because we had maintained frequent dialogues with AT&T’s  International team…that is what saved us in the end. Ultimately we weren’t charged the $500.

Conclusion

Perhaps $500 doesn’t even sound. all that bad to you. It wasn’t really all that bad to us either when compared to another incident a few years back. That time was during a Mediterranean Cruise. My portion of our family plan bill was $13,000. That situation was more of a nightmare to straighten out…but we finally did manage to.

Once again. what saved us were my OCD tendencies to micro-manage our data use while vacationing (although you probably couldn’t tell it from our final result!) I’m telling you this story for one main reason. You will probably  be forgiven for a lot of charges if you act responsibly and check in with your cellular provider periodically as you travel.
